Passport Requirements:
Original, signed passport and one photocopy of information page. The passport must be valid for 6 months beyond stay in Canada, with at least one blank passport page available for Canadian visa stamp. Amendment pages in the back of the passport are not suitable for Canadian visas.
Canada Visa Application Form:
One original Canadian visa application form. After placing your order, fill out the application form included in your Visa Application Kit.
Photo Requirements:
Two recent, identical, 2 x 2 passport photographs, in color, front view and with a plain/light background.
Other Forms:
Family Information Form - One signed original plus one copy. Schedule 1 Form - One signed original plus one copy. Consent Form - One signed original plus one copy. Use of a Representative Form - Two signed originals plus one copy. Letter of Authorization Form - Two signed originals.
Letter Of Invitation:
Original acceptance letter from the educational institution in Canada plus one copy.
Other Requirements:
Letter of intent, respectfully requesting the issuance of a visa and describing the purpose of travel to Canada.

The US State Department strongly encourages American citizens planning international travel to register with the US Embassy(s) in the countries they plan to visit. Registration notifies the US Embassy in each country of your travel plans and enrolls you in the State Department travel advisory and warnings program.
